• NCB / NCD status for your vehicle insurance, either 0%, 25%, 30%, 38.33%, 45% or 55% 
• Vehicle's plat number 
• Your identity card (I/C) number 
• Effective date of NCB for your vehicle 
• Insurance / Takaful companies that cover your vehicle 
• Code No. to print NCB report from the ISM website ( Click Me )
